Product Order Form
Use this form only if you are paying by check, money order or credit card by telephone. If you are paying by credit card or virtual check, please use the proper form.
Please follow these instructions for completing your order.
- Choose a product and quantity for at least the first item; multiple items are optional.
- Include any relevant information or requests about the product(s) you are ordering such as color, trim options, etc.
- Be aware that field names in red text are required to complete your order.
Shipping will not be calculated for international orders. Customers will be contacted by email with the total cost of their order with the applicable shipping included.
Note: In striving to keep our prices as low as possible, hat boxes are not included with your order.
